We previiously sent you an Email on 05/07/2011 regarding on your claim form, you have answered 'Yes' to the question "Is there a Court order or financial agreement that may affect entitlement “?.  I am writing to you to clarify if this is correct? If your answer is yes, could you please forward copies of the agreement/court order to :

As yet we have not received your response. If you have made an error in the application and there is NOT a court order or financial agreement in place, could you please advise by Friday 22/07/2011. If we do not receive an answer by this date we will be unable to process your claim.
